Personalized Medical Care - Primary Care Physician

A. Services Included as Part of Your Annual Retainer Fee:

Same-day or next-business-day appointments.

Little or no office waiting time and extended office appointments.

Strong focus on preventive medicine and long-term health and wellness.

Direct phone access to a physician or medical assistant during office hours.

Personal cell phone access to Drs. Olin and Slogoff.

Convenient fax or e-mail access for non-urgent health issues or questions.

House calls will be available in situations where an illness prevents you from traveling to our office.

Specially trained staff dedicated to your care.

End-of-life counseling visits with patients and family members.

B. Insurance-Related Issues and Personal Physicians of Connecticut, LLC:

Non-Medicare Patients:
Effective April 1, 2007, we have elected not to participate in any commercial health insurance plans. As “out-of-network” physicians, we will no longer collect any co-pays for office visits. Our new charge for medical care rendered during your office or hospital visit will be a flat, per-visit fee. (Please contact our office for more details regarding this per-visit fee.) If your health insurance plan provides coverage for some of our out-of-network services, you may wish to file a claim for our per-visit fee directly with your insurance plan.

Medicare Patients:
Effective April 1, 2007, we have elected not to participate in the Medicare program. Consequently, the Medicare program will not cover our annual fee nor will it cover our flat, per-visit fee. However, it is important to emphasize that services rendered by Medicare-participating providers (e.g., diagnostic imaging studies, laboratory tests, hospitalizations, specialist consultations, etc.) may be covered by Medicare subject to their eligibility requirements.
