A. Services Included as Part of Your Annual Retainer Fee:

Same-day or next-business-day appointments.

Little or no office waiting time and extended office appointments.

Strong focus on preventive medicine and long-term health and wellness.

Direct phone access to Drs. Slogoff and Rand during office hours.

Personal cell phone access to Drs. Slogoff and Rand.

Convenient fax or e-mail access for non-urgent health issues or questions.

House calls are available in situations where an illness prevents a PPC patient from traveling to the office.

Specially trained staff members who are dedicated to providing the highest quality of care for PPC’s patients.

End-of-life counseling visits with patients and family members.

B. Insurance-Related Issues and Personal Physicians of Connecticut, LLC:

Non-Medicare Patients:
Drs. Slogoff and Rand have elected not to participate in any commercial health insurance plans. As “out-of-network” physicians, they do not collect any co-pays for office visits. There is a flat, per-visit fee for medical services rendered during your office or hospital visit. (Please contact the office for more details regarding this per-visit fee.) If your health insurance plan provides coverage for some out-of-network services, you may wish to file a claim for PPC’s per-visit fee directly with your insurance plan.

Medicare Patients:
Drs. Slogoff and Rand have elected not to participate in the Medicare program. Consequently, the Medicare program will not cover PPC’s annual fee nor will it cover PPC’s flat, per-visit fee. However, it is important to emphasize that services rendered outside of PPC’s office by Medicare-participating providers (e.g., diagnostic imaging studies, laboratory tests, hospitalizations, specialist consultations, etc.) may be covered by Medicare subject to their eligibility requirements.
